BastionGPT is designed for HIPAA compliance, and a Business Associate Agreement (BAA) is included by default with every plan, at every tier, including individual practitioners on the lowest paid tier. If you would like a signed copy of the BAA for your records, reach out to the BastionGPT support team and they will provide one. The platform is also designed to align with PIPEDA (Canada), the Australian Privacy Principles (APP), and 42 CFR Part 2 (substance use disorder records). Independent confirmation of the BAA-by-default policy is published by Paubox, HIPAA Times, and Defensorum.

BastionGPT lists three tiers: Professional at $20 per user per month, Professional Plus at $45 per user per month, and Ultra at $65 per user per month. BastionGPT has two substantively negative reviews on record, both reproduced in full on this page and both from the same week in December 2024. An anonymous Capterra reviewer titled their review “Zero customer service” on December 3, 2024, reporting unresolved issues after two meetings with the company. A Reddit post on r/LocalLLaMA from December 2, 2024 described BastionGPT note quality as “fairly atrocious” and speculated the platform was running base GPT-4. Both reviews are roughly 17 months old. Since then, support has been substantially expanded and the platform routes across 200+ models including GPT-4o, Claude, and Gemini. Changes since December 2024 are documented under each review.

BastionGPT is the flagship product of Bastion Intelligence, a brand of FortaTech Security LLC. The company is based in Dallas, Texas, was founded in 2018, and is led by Joshua Spencer, formerly Chief Technology Officer at UT Southwestern Medical Center for 12 years.

Reviewers describe BastionGPT as functionally similar to ChatGPT, Claude, and Gemini, but built for healthcare workflows. The most-cited differences are HIPAA compliance with a BAA included on every plan, no use of clinical inputs for model training, and a prompt library tuned for clinical documentation. Under the hood, BastionGPT routes across 200+ models including GPT, Claude, and Gemini, so you are using the same frontier models as the consumer apps, accessed through a secure healthcare-specific architecture.

A feasibility study published in Critical Care Explorations (Blackman et al., April 2026, 8(4):e1378) used BastionGPT as the chatbot platform in a Baylor College of Medicine / Texas Children's Hospital pediatric ICU communication workflow. The study reported 99.3% sentence-level accuracy across 1,225 AI-generated sentences, with three independent physician reviewers and a Gwet's AC2 inter-rater agreement of 0.98.

Published reviews come primarily from clinical psychologists, school and educational psychologists, pediatric physicians, and nurses. Vendor-published testimonials and case studies expand the list to include dentists, therapists, and medical coders. The peer-reviewed study used BastionGPT in a pediatric intensive care unit.

The evidence base for solo practitioners is positive but small. The strongest published evidence comes from clinical and school psychologists who report substantial time savings on report writing. The free trial lets you test fit before committing.

BastionGPT is designed not to retain memory across encounters. That is a HIPAA-aligned design choice, not a missing feature: persistent cross-session memory would create a path for one patient's information to surface in another patient's session. The intended pattern is a saved prompt that includes your reference materials, vocabulary, format preferences, and context, re-applied each session.
