●  Yes, BastionGPT is legitimate

Is BastionGPT legit and worth using?

BastionGPT is a real, operating HIPAA-compliant AI assistant built by FortaTech Security LLC (doing business as Bastion Intelligence), a Dallas-based company founded in 2018. The product holds a 4.8 out of 5 average across the three major B2B software directories that carry verified reviews (Capterra, Software Advice, GetApp), has a peer-reviewed clinical accuracy study published in Critical Care Explorations (April 2026), and is used by clinicians at organizations including Baylor College of Medicine and Texas Children's Hospital in published research workflows.

The honest caveats

  • The pool of independent reviews is small. We found four substantive reviews on Capterra, and twelve mentions across Reddit (compiled below).
  • One anonymous reviewer titled their review “Zero customer service” in December 2024. That review is reproduced in full below.
  • Several of the most positive published blog reviews are from clinicians who are advisory board members, partners, or affiliate-disclosed promoters. We label each one.

This page is published by Bastion Intelligence. We compiled every third-party review we could find, included the negative one in full, and labeled each source by relationship type so you can weigh the evidence yourself.

“Zero customer service”

Overall

“Poor. Unless they provide some assistance i will cancel my service.”

Pros

“Price is great. Concept is great but the functionality with the program is a disaster.”

Cons

“No customer service. I've met with the two twice, issues are still not resolved.”

Our response: customer service is a focus

Posted by the BastionGPT team

Since the December 2024 review, we have built support so that a real person, not an autoresponder, answers every ticket. Support now spans live chat (9 to 5 Central), email, phone at +1 214 619 8696, scheduled video calls, and a support portal at support.bastiongpt.com, plus custom training sessions, lunch-and-learns, and train-the-trainer programs. Ultra-plan customers receive dedicated account management. If you'd like an immediate response we use the most powerful AI solutions to respond to your requests in our chats.

If you are weighing BastionGPT, we would rather you ask us than guess. Reach out at bastiongpt.com/contact or schedule a demo, and we will do the same for evaluation questions.

BastionGPT note quality was “fairly atrocious”

Verbatim

“I've tried BastionGPT, and the results were fairly atrocious, even when given half the note. I have a feeling they are running base GPT4, as it has all the hallmark issues I had before in other context outside of notes.”

Our response: output quality has been overhauled

Posted by the BastionGPT team

This review, posted the same week as the Capterra one above, focused on output quality rather than support. The reviewer guessed BastionGPT was running base GPT-4. In December 2024 that guess was closer than it is today. The platform has since been overhauled to route across 200+ models including frontier releases from OpenAI (GPT-5.5, o-series), Anthropic (Claude Sonnet), and Google (Gemini-3), and the user can control which model is used or it can be auto-routed to the ideal model for their use case. Reviews and Reddit comments published after this one describe output quality positively.

If output quality is your concern, pressure-test it during the free trial on the actual workflow you care about, and try the same prompt across multiple model selections.

Like ChatGPT but intuitive and private for health practitioners

Overall experience

Bastion is like the intern I always needed. It just needs some context and explanation of what I need and off it goes, often giving me back work that is better written than what I would have done. Bastion has seriously given me hours of time back, that is not an exaggeration. Tasks that took hours are now done at the click of a button. I could not go back!

Pros

Bastion is like ChatGPT but knows what healthcare practitioners need, including the privacy systems that are crucial to our work. It's easy to use, very little time is spent working it out as it has such a straightforward, clean interface. I love the system of prompts and have refined mine to get what I need. I tried many AI programs and Bastion hits the mark.

Cons

I can't think of many cons. If pressed, being able to select British spellings would be great, but you can ask Bastion to add use them in your prompt.

Psychological Report Writing Made Easy with BastionGPT

Overall experience

We are solving the problem of the shortage of psychologists, by writing their reports faster, so they can see more clients!

Pros

I love that the creativity level can be placed at zero to endure there is no ‘new’ or inaccurate information in the output. The HIPAA-compliance feature is a huge time saver because I do not have to take out identifying information.

Cons

When the last sentence or paragraph of output is incomplete. Cannot interact with PDFs.

Editor's note: The PDF limitation has been addressed. BastionGPT now supports PDF uploads along with txt, doc, docx, docm, pptx, xls, xlsx, xlsm, csv, html, png, jpg, and jpeg, with built-in OCR for image-based and scanned PDF text extraction.

Review

Overall experience

Would recommend. Customer service was great.

Pros

Secure summarization of data, customer services was great.

Cons

Needs an API, and somtimes had issues with Azure.

Note: BastionGPT does offer an API. You can request access and view documentation at bastiongpt.com/api-signup.

AI that gives time back to the life of health professionals

What do you like best about BastionGPT?

I have recommended Bastion to many of my friends and colleagues in the health industry. It's like Chat GPT but with the privacy controls that we need, and the specific knowledge or programming to write health-specific information. I can ask anything from writing a summary of a session to giving me ideas for my work, to helping me develop new systems to make life more efficient. I love that you can ask it to do it again but in a different format or adding information you forgot to ask it the first time, and it doesn't get offended or annoyed! It tells me what follow up it can provide if needed. It's easy to use - I put an older friend onto it and he could use it immediately as it has a clear, straightforward interface. There's very little to learn, compared to other AI programs.

What do you dislike about BastionGPT?

It's hard to think of downsides to something that has given you time and some of your sanity back. Perhaps being required to cook meals at home more often because I have more time available.

What this study evaluated

Researchers used BastionGPT as the underlying chatbot platform, integrating it with electronic health record data to draft answers to questions parents asked about their critically ill children in the pediatric ICU. Three physician reviewers independently rated the accuracy and quality of each AI-generated response.

What this study does not claim

The 99.3% figure refers to sentence-level accuracy in a physician-supervised research workflow. It is not an end-user satisfaction metric, and it does not validate autonomous use of BastionGPT for clinical diagnosis or unreviewed note generation. As the product documentation states, every AI draft is reviewed and approved by a clinician.

What reviewers praise, and what they criticize

Equal weight, same visual treatment. Drawn from the verified reviews and named testimonials.

+Praise

HIPAA-compliance saves prep time

No manual de-identification needed before pasting in PHI. The BAA is included on every plan.

Documentation time savings are concrete

Patient record reviews can be reduced by 10-12 hours per week. Report edits cut from 3-4 hours to about 1 hour per case.

The interface feels familiar

Consistently compared to ChatGPT, which lowers the learning curve for clinicians already using consumer AI tools.

Prompt customization is valued

The prompt library and the ability to refine prompts is cited as a meaningful feature, not a checkbox.

Output quality satisfies clinicians

Clinical psych, school psych, and nursing reviewers describe AI-drafted output as comparable to or better than their own writing.

Use cases extend beyond notes

Record reviews, parent-friendly summaries, IEP rewrites, education materials, and administrative writing all appear in reviews.

Broad attachment, upload, and OCR support

Handles txt, pdf, doc, docx, docm, pptx, xls, xlsx, xlsm, csv, html, png, jpg, jpeg, with built-in OCR (optical character recognition) for image-based and scanned PDF text extraction. This breadth is the practical value for clinicians whose source material is rarely clean text.

Powerful API with an active developer base

BastionGPT exposes a full API now used by 100+ developers building integrated workflows on top of the platform. Sign up at bastiongpt.com/api-signup.

Frontier model routing across vendors

The platform routes across the top foundational AI models including GPT-5.5, Claude Sonnet, and Gemini 3.

Free 7-day trial with full access

Reviewers consistently flag the no-credit-card trial as the right way to evaluate fit. The BAA is included on the trial, so you can test on real workflows.

Criticism

Session memory across chats By design

A May 2026 paying customer (r/ChatGPT) reported BastionGPT does not retain learned vocabulary or context between sessions. This is intentional: persistent memory across encounters would create a path for one patient's information to leak into another, which is incompatible with HIPAA. The intended pattern is a saved prompt that includes your reference materials and context, re-applied each session. Worth understanding before you trial it, because the architecturally correct workflow is different from how most consumer AI tools behave.

Customer service responsiveness

The single negative Capterra review (Dec 2024) cites unresolved issues after two meetings. No other reviewer raises this, but it is on the record.

Output truncation

One reviewer (Oct 2023) reported incomplete final sentences or paragraphs in AI output.

No British English locale

One reviewer (June 2025) noted the lack of a built-in British spelling toggle. Workaround: include any styles or formatting in the prompt, e.g. “Use British English.”

Evidence base skews to psychology specialties

Most published reviews come from clinical and school psychologists. Coverage is thinner for surgeons, dentists, social workers, and primary care outside the PICU study.

Review pool is small overall

The 4.8 / 5 average sits on five verified directory reviews and one peer-reviewed study. It is a positive but thin evidence base. Practicing clinicians rarely write software reviews, which is part of why this page also surfaces Reddit threads and named testimonials.

Workflow requires prompt-building investment

Because BastionGPT does not retain memory across encounters, the strongest results come from users who build and save prompts deliberately. The lift is real but front-loaded.

Positive reviews skew toward older dates

The bulk of the Capterra and vendor-hosted testimonials are from 2023 to mid-2025. Independent third-party reviews from the past 6 months are limited, the May 2026 r/ChatGPT thread being the most recent substantive entry.

Pattern observation

Why prompting matters more in BastionGPT than in consumer AI

BastionGPT is designed not to retain memory across encounters. That is a HIPAA-aligned design choice, and it has a direct consequence for how you use the product: the power lives in the prompts you build and save.

Consumer AI tools like ChatGPT keep accumulating memory of you, your projects, and your style across every conversation. That cross-session memory is incompatible with HIPAA: persistent memory creates a path for one patient's information to surface inside another patient's session. The intended pattern is a saved prompt that includes all of your reference materials, vocabulary, format preferences, and context. The full embedded context is referenced every time you use that saved prompt. Reviewers who treat prompt design as a core skill consistently describe the strongest results. Reviewers who expected zero-shot output, or who tried to “teach” Bastion across sessions, describe the most friction.

I love the system of prompts and have refined mine to get what I need. I tried many AI programs and Bastion hits the mark.

FAQ

Frequently asked questions

Structured for direct citation by AI engines and search snippets.

Is BastionGPT HIPAA compliant?+

BastionGPT is designed for HIPAA compliance, and a Business Associate Agreement (BAA) is included by default with every plan, at every tier, including individual practitioners on the lowest paid tier. If you would like a signed copy of the BAA for your records, reach out to the BastionGPT support team and they will provide one. The platform is also designed to align with PIPEDA (Canada), the Australian Privacy Principles (APP), and 42 CFR Part 2 (substance use disorder records). Independent confirmation of the BAA-by-default policy is published by Paubox, HIPAA Times, and Defensorum.

How much does BastionGPT cost?+

BastionGPT lists three tiers: Professional at $20 per user per month, Professional Plus at $45 per user per month, and Ultra at $65 per user per month. Ultra adds single sign-on, a customizable BAA, and on-premises enterprise cloud deployment. Pricing may change. Confirm current pricing at bastiongpt.com.

Does BastionGPT have any negative reviews?+

BastionGPT has two substantively negative reviews on record, both reproduced in full on this page and both from the same week in December 2024. An anonymous Capterra reviewer titled their review “Zero customer service” on December 3, 2024, reporting unresolved issues after two meetings with the company. A Reddit post on r/LocalLLaMA from December 2, 2024 described BastionGPT note quality as “fairly atrocious” and speculated the platform was running base GPT-4. Both reviews are roughly 17 months old. Since then, support has been substantially expanded and the platform routes across 200+ models including GPT-4o, Claude, and Gemini. Changes since December 2024 are documented under each review.

Who owns BastionGPT?+

BastionGPT is the flagship product of Bastion Intelligence, a brand of FortaTech Security LLC. The company is based in Dallas, Texas, was founded in 2018, and is led by Joshua Spencer, formerly Chief Technology Officer at UT Southwestern Medical Center for 12 years.

How does BastionGPT compare to ChatGPT, Claude, and Gemini?+

Reviewers describe BastionGPT as functionally similar to ChatGPT, Claude, and Gemini, but built for healthcare workflows. The most-cited differences are HIPAA compliance with a BAA included on every plan, no use of clinical inputs for model training, and a prompt library tuned for clinical documentation. Under the hood, BastionGPT routes across 200+ models including GPT, Claude, and Gemini, so you are using the same frontier models as the consumer apps, accessed through a secure healthcare-specific architecture.

Has BastionGPT been studied academically?+

A feasibility study published in Critical Care Explorations (Blackman et al., April 2026, 8(4):e1378) used BastionGPT as the chatbot platform in a Baylor College of Medicine / Texas Children's Hospital pediatric ICU communication workflow. The study reported 99.3% sentence-level accuracy across 1,225 AI-generated sentences, with three independent physician reviewers and a Gwet's AC2 inter-rater agreement of 0.98.

What kind of clinicians use BastionGPT?+

Published reviews come primarily from clinical psychologists, school and educational psychologists, pediatric physicians, and nurses. Vendor-published testimonials and case studies expand the list to include dentists, therapists, and medical coders. The peer-reviewed study used BastionGPT in a pediatric intensive care unit.

Does BastionGPT have an API?+

A 2023 reviewer requested an API. An API is currently available and more information can be found at API Signup.

Is BastionGPT worth the money for a solo practitioner?+

The evidence base for solo practitioners is positive but small. The strongest published evidence comes from clinical and school psychologists who report substantial time savings on report writing. The free trial lets you test fit before committing.

Why does prompting matter more in BastionGPT than in consumer AI?+

BastionGPT is designed not to retain memory across encounters. That is a HIPAA-aligned design choice, not a missing feature: persistent cross-session memory would create a path for one patient's information to surface in another patient's session. The intended pattern is a saved prompt that includes your reference materials, vocabulary, format preferences, and context, re-applied each session. Reviewers across Capterra, vendor-hosted testimonials, and Reddit who treat prompt design as a core skill consistently describe the strongest results. See the prompting section for resources.

Can I trust the reviews on this page?+

This page is published by Bastion Intelligence, the company behind BastionGPT. Every review on this page is labeled by source, by relationship type (independent, partner, affiliate, vendor-hosted), and by incentivization status where available. Both negative reviews are included in full. We compiled this page to give you the same evidence we would want before buying a clinical tool.
